Residential Properties:

1.  Print the attached Appointment of Agency Form.
        This form authorizes us to represent you at the Appraisal         District.

• Fill in Steps 1 (name and mailing address), Step 2 (property address or parcel ID if known)

• Sign your signature on Page 2, Step 6

2.  Print the attached Property Tax Questionnaire

• Pictures are very important, if there are condition problems!

Commercial Properties:

• Prior Year's Balance Sheet and Income Statement
• Current YTD Income Statement
• Prior Year's Tax Return
• Current rent roll

Return the completed forms to us by Fax (214-447-9392), Email, or mail.

Upon receipt, we will send you a confirmation that we have received the documents. If you do not receive our confirmation in a reasonable time after transmitting please call us.
